{"id":6506,"date":"2018-10-11T08:45:27","date_gmt":"2018-10-11T03:15:27","guid":{"rendered":"http:\/\/calamur.org\/gargi\/?p=6506"},"modified":"2019-04-15T02:16:38","modified_gmt":"2019-04-15T02:16:38","slug":"cnbc-tv18-column-ayushman-bharat-where-are-the-doctors","status":"publish","type":"post","link":"https:\/\/calamur.org\/gargi\/2018\/10\/11\/cnbc-tv18-column-ayushman-bharat-where-are-the-doctors\/","title":{"rendered":"CNBC TV18 Column: Ayushman Bharat &#8211; Where are the Doctors?"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">I write for <a href=\"https:\/\/www.cnbctv18.com\/healthcare\/ayushman-bharat-where-are-the-doctors-931051.htm\"><strong>CNBC TV18 on Ayushman Bharat<\/strong><\/a><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the years since liberalisation,\nIndia has added over 500 million new citizens. the country itself has gone from\nbeing an economic basket case, to being the 6<sup>th<\/sup> largest economy in\nthe world <strong><a href=\"https:\/\/www.bloomberg.com\/news\/articles\/2018-08-08\/india-s-economy-is-an-elephant-that-s-starting-to-run-imf-says\">at\nUS$ 2.6 trillion<\/a><\/strong>. Poverty alleviation has been huge,&nbsp; with over <strong><a href=\"https:\/\/www.ndtv.com\/india-news\/over-10-years-poverty-rate-in-india-reduced-to-half-un-report-1919756\">270\nmillion people<\/a><\/strong> being pulled out of poverty in the last decade\nalone. However, one crucial aspect that could mar the India growth story &nbsp;is inadequate healthcare and health coverage. &nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Less than 1.3% of the total GDP of\nIndia was spent on healthcare. Which means that sudden illness becomes the <strong><a href=\"http:\/\/apps.who.int\/iris\/bitstream\/handle\/10665\/259642\/HFP-IND.pdf?sequence=1&amp;isAllowed=y\">single\nbiggest out of pocket expense<\/a><\/strong> for Indian families. &nbsp;The situation is bad enough for the middle\nclass, for the poor it is devastating. WHO calls poor families coping with\nchronic illness as a financial catastrophe; in the last decade their health\ncare costs have <strong><a href=\"http:\/\/www.who.int\/bulletin\/online_first\/BLT.17.191759.pdf\">increased\nthreefold.<\/a><\/strong> &nbsp;This financial\ncatastrophe is especially acute for poor families with older members. It is\nestimated that 63 million people, or <strong><a href=\"https:\/\/cddep.org\/blog\/posts\/63_million_indians_are_pushed_poverty_health_expenses_each_year-and_drugs_are_chief_cause\/\">7%\nof the Indian population slip back into poverty<\/a><\/strong>trying to manage health care costs..\nIt is in this context that one needs to see the largest medical coverage\nprogramme in the world \u2013 Ayushman Bharat or the &nbsp;Ayushman Bharat \u2013 Pradhan Mantri Jan Aarogya\nYojana (AB-PMJAY),<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ayushman Bharat, dubbed Modicare,\nprovides health cover to over 500 million people, or a 100 million families \u2013\nfor sums up to Rs 5 lakhs, for hospitalisation. In addition to the stay at the\nhospital, the PMJAY covers 2 days of pre-operation, 15 days of post operation,\nas well as the cost of medicines and diagnostics in this period. &nbsp;In a country where the interiors and rural\nareas are marked by a lack of high-end healthcare, necessitating travelling to\nlarger medical centres with better facilities and doctors, PMJAY offers\nnationwide portability.&nbsp; This means a\ncovered patient from a remote rural area is entitled to their health cover being\nvalid in an empanelled speciality hospital. There is going to a boom in the\nhealthcare sector, as 10 crore families begin to get covered. The programme\nthat covers a population larger than that of most countries, is envisaged as the\nModi Government\u2019s flagship programme \u2013 one that they hope will bring them\nelectoral dividends<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The biggest practical hurdle the\nPMJAY is going to face is the acute shortage of doctors in India. As per the\nWorld Bank (2014), India has <strong><a href=\"https:\/\/data.worldbank.org\/indicator\/sh.med.phys.zs\">8\nphysicians for every 10,000 people<\/a><\/strong> &#8211; &nbsp;the world average is 15 doctors per 10,000\npeople. There were just <strong><a href=\"http:\/\/www.ijph.in\/article.asp?issn=0019-557X;year=2017;volume=61;issue=3;spage=182;epage=187;aulast=Potnuru\">over\n9.4 lakh doctors<\/a><\/strong> registered with the Medical Council of India, a\nhigh proportion&nbsp; of them concentrated in\nthe big cities. Medical bodies are unclear as to how many of these are\npractising, and how many have emigrated to work outside India. some studies put\nthe effective figure as low as <strong><a href=\"http:\/\/www.ijph.in\/article.asp?issn=0019-557X;year=2017;volume=61;issue=3;spage=182;epage=187;aulast=Potnuru#ref3\">4.8\ndoctors per 10,000<\/a><\/strong> people. It is these low numbers that are\ngoing to be an implementation hurdle.&nbsp; <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&nbsp;From 19 medical colleges at the time of\nIndependence, India now has <strong><a href=\"http:\/\/www.ijph.in\/article.asp?issn=0019-557X;year=2017;volume=61;issue=3;spage=182;epage=187;aulast=Potnuru#ref3\">412\nmedical colleges,<\/a><\/strong> over 57% of which are privately owned, yet we\nare unable to produce the number of doctors needed to keep India healthy and\nticking. It is estimated that this short supply of doctors is going to get more\nacute. By 2030, we are going to need <strong><a href=\"http:\/\/www.ijph.in\/article.asp?issn=0019-557X;year=2017;volume=61;issue=3;spage=182;epage=187;aulast=Potnuru#ref3\">2.07\nmillion more doctors<\/a><\/strong> to be able to provide adequate healthcare\nfor all. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And, this is where the Modi\ngovernment should look at innovative and disruptive steps to meet the need. One\nwould be to increase the number of medical colleges, and teaching hospitals.\nBut, that is going to take time. You are looking at least a decade long window\nbefore results start showing. The second is a massive push in public healthcare,\nupgrading and expanding the creaking system. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The third, would be to look at the\nprimary and secondary health care sector, and borrow a page from how western\ncountries built up their healthcare systems. They did that by allowing foreign\ndoctors to pass an exam and practise in their country. They offered them a\nbetter opportunity that they would have in their country. And, as a result, the\nwest scaled up fast on the backs of immigrant doctors. India should follow\nsuit. To make sure that PMJAY is a success, we need doctors. And, the only way\nin which we can scale up fast enough to meet the demand for healthcare, is with\nmigrant doctors. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ayushman Bharat is a welcome move\nby the Government to provide a layer of health cover for the most vulnerable. It\npromises to invigorate the health care economy.
